How long was it before this recipe started going south? and how long before it was undrinkable?
Any thoughts on why this may have happened?

Sorry for the delay in replying. I've been laid up after back surgery.
Looking at the tasting notes, it looks like it was OK for the first two years, and by the time another two years had come around it was gone.
As for the reason, I can't hazard a guess. It wouldn't have been kept in ideal conditions (i.e. it was in the shed), but other beers stored alongside it seem to have fared far better.
